In this folder you will find the PowerPoint slides presented at the 2015 Wisconsin FGMI, as well as the binder of all session documents. A variety of sessions were offered at this Institute, all of which were designed to increase understanding of financial and grants management. We encourage you to take full advantage of these resources that can help you manage your program effectively.

Each member must have this form completed, including member and program staff signatures, with signature dates that are on or before the member's start date.

Criminal History Check Important Documents

All employees, participants, and others who receive a salary, national service education award, living allowance, or stipend under CNCS grants, even if the activities don't involve service with vulnerable populations, must receive the checks prior to beginning employment or service.

The following checks must be completed on or before each member's start date in order to ensure eligibility.
Missing, incomplete, or late checks can cause hours and associated CNCS funds to be disallowed.

  1. Wisconsin State Criminal History Registry Check (and state of residence at time of application if member resided outside of WI)

  2. National Sex Offender Public Registry (NSOPR) check

  3. Wisconsin Circuit Court Access (WCCA) check

  4. FBI Fingerprint Check**

*FBI & WI State Criminal History Registry checks must be at least initiated by start date - maintain documentation of this status in member files.

**FBI check only required for those who will have reoccurring access to vulnerable populations.

Monitoring Tools

The documents below are tools used by Serve Wisconsin Program Officers in monitoring of AmeriCorps State sub-grantees. It is highly recommended that programs review these forms so that you are completely prepared for your monitoring visits. Contact your Program Officer with any questions. These forms may change slightly from year to year.

Health Insurance Options

Most Wisconsin AmeriCorps programs use healthcare policies through the The Corps Network

  • Available to AmeriCorps national service programs in Wisconsin because Serve Wisconsin is a Corps Network member

The Corps Network is the most commonly used policy by Wisconsin AmeriCorps programs. A couple of organizations use their own health care providers, but The Corps Network offers plans that cover the minimum requirements at a reasonable rate. Serve Wisconsin does not endorse any particular provider. Health insurance offered to AmeriCorps members must meet minimum requirements specified in AmeriCorps regulations.

Additionally, The National Service Knowledge Network offers a collection of resources and sample documents designed to provide ideas and templates for creating your own forms. CNCS also offers a range of CNCS forms for your use.
